Primary Care Liaison

The aim of the Primary Care Liasion Project is to expand the links between the voluntary sector and all primary care staff. We act as a signposting service to various local and national voluntary and community organisations that may be able to help the healthcare professional's patients.

There are many voluntary organisations that are able to provide support and information on a wide variety of subjects. We have access to over 2000 voluntary and community organisations, both national and locally based. They are able to offer practical and emotional support and some of the areas that they are able to assist with include:

  • Advocacy
  • Alcohol and drug misuse
  • Bereavement
  • Carers
  • Isolation
  • Physical disabilities and a range of specific disabilities
  • Sensory impairment
  • Specific medical conditions

The list is endless.........

Examples of referrals received include:

  • carer of a wife with Alzheimer's seeking information and day-to-day support
  • Facilities and activities available in the Wrexham area for an elderly gentleman feeling isolated
  • Information on local organisations able to provide support and social activities following a heart operation.
