Title: Community Agent Job Description
Hours of work: Up to 16 hours per week
Salary Range: £12.00 hourly
Place of Work: Home and Community of Assignment, Ty AVOW (Wrexham).
Location of Preference to be indicated on application form: Erbistock, Esclusham, Marchwiel, Llangollen Rural and Sesswick. Location to be assigned at time of offer.
Responsible to: Community Development Team Manager
Contract: 31st March 2025, with possible annual extension based on continued funding.

Context
AVOW is recruiting a team of Community Agents seeking to ensure that individuals aged 50+ have access to the services and support they need. The Community Agents are managed by AVOW in partnership with the Clerk of the Community Council.
Community Agents are an innovative response to a recognised need that many people living in our communities are unable to connect with key services that help and maintain well- being and support their quality of life. Community Agents provide access to a wide range of information that will enable clients to make informed choices about their present and future needs. The aim is to help older people feel more independent, secure, and cared for, and to have a better quality of life. Community Agents will support people from across Wrexham, bridging the gap between the local community, statutory services and voluntary organisations.
Responsibilities:
• To support the people aged 50+, in their community, to live the life they want, to be part of the community: to feel good, stay well and keep safe.
• To provide high quality face-to-face information, advice and support, helping clients to make informed choices about their needs and goals.
• Engage with clients in a friendly, courteous and appropriate manner, using excellent communication skills to deliver high-quality customer service.
• To participate in and facilitate events within the community.
• Work in both a reactive and proactive way to create a welcoming community atmosphere while meeting the needs of multiple clients.
• Accurately maintain electronic client records, calls and referrals complying with Data Protections Act: use IT equipment appropriately and effectively.
• Prepare monthly progress report for WCBC and occasional reports to the Community Council(s).
• To work in accordance with the aims and objectives of the Community Agent scheme and its standards of conduct and practice.
• Adhere to all health and safety requirements both in the home and in the communities taking reasonable care not to do anything that may endanger yourself or others.
• To identify existing community support (e.g. voluntary organisations, care schemes, informal networks) to address any gaps as appropriate.
• To develop working relationships with service providers to advocate and facilitate appropriate service support for vulnerable people and their families.
• Undertakes continuous professional development and training, constructively participating in meetings, supervision and other events designed to improve communication and assist with personal skills development within the role.
• Maintains confidentially of all information and records and work undertaken for monitoring and evaluation purposes.
Lleoliad
• The post will be based within the Community Council Areas specified at the time of contract. Home working will be encouraged when not in the locality with support from AVOW head office at Ty Avow, Wrexham.
• The post-holder may be required to work at locations across Wrexham to support the Community Agent programme delivery.
Additional Requirements:
• Other responsibilities, which may be negotiated from time to time, which are commensurate with the level and nature of the post. The post will require a degree of flexibility.
• While a driving license is considered essential. We will consider other modes of
transportation as long as the agent has the ability to visit different sites within the Community(ies) which is essential as part of this job role.
